Your friendly neighborhood surgeon here. The pain is caused by a change in barometric pressure. When the air pressure goes down, the nitrogen dissolved in closed body compartments swells. The baroreceptor nerve endings detect this, and the brain converts their signals into pain sensations. The ankle I broke in college does the same thing every time there is a storm brewing.
